It was downloaded from http://www.fcusack.com/
The patch for Heimdal came from http://ns1.logidee.com/~joko/heimdal/src/
The unused FreeBSD patches came from http://www.freebsd.org/cgi/cvsweb.cgi/ports/security/pam_krb5/files/

Not really sure. I described problems on this mailing list, and was
told "use this patch". It probably should be somewhere on this mailing
lists archives.
